Traumatic Intramural Hematoma of the Duodenum



Abstract
Intramural hematoma of the duodenum is a clinical entity which has been recognized with increasing frequency during the past two decades. The authors present a typical case in 18--year-old man who had been struck by a foot-ball in his abdomen.
The diagnosis should be, made clinically and radiologically in a. major percentages of cases. The classical "coil-spring" sign seen on upper gastrointestinal roentgenogram can probably be attributed to partial in tussusception of small. bowel distal to the hematoma. The lesion is most -successfully managed by surgical intervention and simple evacuation of the hematoma with or without added gastrojejunostomy. Certain instances in the presence of clotting defects or with only minimally duodenal obstruction may be given a trial of conservative management with nasogastric, suction and fluid replacement.
